DUTIES & RESPONSIBILITIES
Before start any task on site, ensure that (H I P) Hazard Identification Plan or (J
S A) Job Safety Analysis must be prepared to prevent the hazard.
Inspecting the work site and taking suitable steps to eliminate all the unsafe
working condition and potential hazards and to maintain good housekeeping.
Preparation of the job safety analysis and hazard identification
Make a joint job site inspection.
Assure that hazardous situations do not arise.
Establish an effective communication with issuing authorities.
The issuer must see to it that receiver certificate is valid for the approved type of work permit such as; hot work, cold work, confined space & gas release.
The issuer & receiver must discuss the need of the work permits.
The issuer & receiver are responsible for the safety of the people and the equipment.
The issuer is responsible for specifying the precaution so that work can be done without accident.
The receiver is responsible in performing the work in accordance with the compliance as stated in the work permit.
Assure that all required firefighting equipment is available and in working condition prior to the start of the job.
Assure that all permit conditions are maintained.
Stop the job when the plant alarm sounds.
Stop the job and notify FW AL Task Area Supervisor if flammable.
Material is released in the immediate area.
Upon job completion, he remains with the job until cleanup is complete.
